Waiver of Lawsuit/Release of Liability

ACKNOWLEDGMENT OF RISK:
I acknowledge that attending the SETBP1 Family & Research Conference (the “Conference”) involves inherent risks, including but not limited to exposure to communicable diseases such as COVID-19, influenza, RSV, and other viral or bacterial infections. I understand that such exposure may occur during the Conference and during related travel to, from, or within the Conference location.

I understand that exposure may result from the actions, omissions, or negligence of other attendees or third parties. I further acknowledge that the SETBP1 Society (“Society”) does not guarantee that exposure to illness will not occur and makes no representation that any safety measures implemented will eliminate or fully reduce such risk.

ASSUMPTION OF RISK:
I have read and understand the risks described above. I voluntarily choose to attend the Conference and assume full responsibility for any and all risks to my health, including illness, injury, disability, or death, that may arise from attendance.

I acknowledge that participation in the Conference is voluntary and that I am responsible for my own health, safety, and conduct during attendance.

WAIVER OF LAWSUIT/ RELEASE OF LIABILITY:
On behalf of myself, my heirs, executors, administrators, successors, and assigns, I hereby fully and forever release, waive, discharge, and covenant not to sue the Society, its board members, officers, employees, volunteers, contractors, agents, affiliates, and representatives (collectively, the “Released Parties”) from any and all claims, liabilities, demands, actions, or causes of action of any kind, whether known or unknown, arising out of or related to my attendance at or participation in the Conference.

This release includes, without limitation, any claims for personal injury, illness, disability, death, property damage, or financial loss, whether caused by negligence or otherwise, to the fullest extent permitted by law.

INDEMNIFICATION:
I agree to indemnify, defend, and hold harmless the Released Parties from and against any and all claims, demands, damages, costs, expenses, judgments, losses, or liabilities (including reasonable attorney fees) arising out of or related to:
– My attendance or participation in the Conference, and/or
– Any claim brought by a third party related to my actions or conduct at the Conference.

SEVERABILITY:
If any portion of this waiver is found to be invalid or unenforceable, the remaining provisions shall continue in full force and effect.

ACKNOWLEDGMENT OF UNDERSTANDING:
I confirm that I have read this Waiver of Liability, Assumption of Risk, and Indemnification Agreement in full, understand its terms, and voluntarily agree to its provisions. I understand that I am giving up certain legal rights, including the right to sue.
