I, the undersigned parent or legal guardian, understand and acknowledge that SETBP1 Society is providing optional, short-term childcare during the SETBP1 Family & Research Conference as a convenience to families. I understand that this is not a licensed childcare facility and that childcare is provided by volunteers and/or contracted providers.

I understand that childcare services are available only while I am present in the hotel and that I must remain reachable at all times. If my child becomes inconsolable, ill, or engages in unsafe behavior, I agree to immediately return to the childcare room when contacted.

I acknowledge that participation in childcare involves inherent risks of minor injury or illness. I voluntarily assume all such risks on behalf of my child.

I hereby hold harmless, waive, and release SETBP1 Society and its childcare volunteers, workers, officers, representatives, agents, organizers, and successors from any and all claims, demands, suits, costs, and charges arising from personal injury or property damage occurring while my child is in their care, except only for loss, harm, or injury caused by gross negligence or intentional misconduct.

SETBP1 Society childcare staff and volunteers will not administer routine or scheduled medications unless specifically arranged in advance and agreed upon in writing.

I authorize SETBP1 Society childcare volunteers to provide basic first aid and to contact emergency medical services or seek emergency medical treatment for my child if needed and if I cannot be reached. I understand that emergency medical personnel may administer medications or treatment as medically necessary. I agree to be responsible for any resulting medical expenses.

I have read, understand, and agree to follow the Childcare Policies and Procedures of SETBP1 Society.
